DOT Physicals: $45.00:
Required for:
*Driving vehicles < 26,000 pounds
*Interstate material transportation
*Transportation of hazardous materials
*Driving a bus with > 15 passenger capacity

The DOT Physical exam includes:
* Medical history
*Physical exam with urinalysis
*Visual and auditory acuity tests
*Blood pressure, cardiovascular, pulmonary exam
*Referrals as appropriate for deviations from normal findings

Physical exams are performed here in accordance with the department of transportation regulations. Typically, these are available on walk-in basis with minimal waiting. A copy of the completed physical form and the Medical Examiner's  Certificate are provided after the exam. Records of these exams are kept in a secure location for two years in accordance with our office privacy policy and DOT regulations.

Pre-Employment Physicals: $40.00
Includes the DOT criteria above along with an ergonomic assessment to ascertain whether a candidate for employment can actually do the work to be assigned. Potential employees should be carefully screened for the physical ability to perform tasks required in the job description. a description of the job requirements is helpful in tailoring the exam to fit the needs of the employer. If pre-existing physical problems are identified, it helps the employer to make good hiring desicions.
